MAYSI-2 Registration

To register your MAYSI-2 manual, please fill out the form located at the front of the MAYSI-2 User's Manual and Technical Report. This manual is a three-ring binder. After you have completed the form, please fax this form to the NYSAP Project Manager at (508) 856-6805. You must complete the entire form and sign and date on the signature of user line located at the bottom of the form. Please keep a copy the form as it is proof that you have registered your MAYSI-2 manual.

Once you have registered your manual, then you are allowed to photocopy the MAYSI-2 Questionnaire and Scoring Forms; however, you are not allowed to photocopy the MAYSI-2 manual. You are also not allowed to distribute the MAYSI-2 forms in any way, or to put the forms on the internet. Any facility that uses the MAYSI-2 must be registered to use it, or this is a violation of the copyright.

MAYSIWARE Registration
